Your plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We will review our payment options to accommodate your financial needs. We accept Visa, MasterCard, American Express and Discover Card for payment.
We will submit all dental and medical insurance claims for you as a courtesy. We will attempt to help you receive full insurance benefits; however, you are personally responsible for your account. Please contact your insurance company to verify if our office is a preferred provider and with any questions you may have regarding your coverage. Patients with insurance are required to pay a 35% down payment; otherwise charges are due in full at the time of service.
Our practice accepts CareCredit, a convenient and flexible financing option to cover out-of-pocket healthcare expenses. It’s easy to apply for CareCredit and you’ll receive an answer almost immediately. If you are approved, you can begin treatment before you get your card (helping you get the care you want, without delay).

Your scheduled appointment time has been reserved specifically for you. We request 24-hours notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment. After missing your second appointment without notifying us 24 hours in advance, you may be subject to being charged an additional fee.
